Transcript Requests

If you signed up (and if applicable, paid) for the college credit option during your course enrollment, please contact the appropriate college as listed below. (Signing up for college credit is not available retroactively.)

  •  For the Emergency Medical Technician (EMT) Program, EMT Recertification, the UCLA-Daniel Freeman Paramedic Program (effective for courses only since 2005), Emergency Department Technician (EDT) Programs contact: West Los Angeles College at 310.287.4388 or visit their web site at: www.wlac.edu (under Student Services, Grades/ Transcripts).
  •  For college credit transcript for the Paramedic Preparation Program visit the El Camino College web site at www.elcamino.edu or in person at El Camino Community College Office of Admissions and Records at El Camino College, 16007 Crenshaw Blvd., Torrance, CA 90506.

To order your transcripts for UCLA Center For Prehospital Care Programs use the Transcript Request Form. The form which includes a "Frequently Asked Questions" section that includes information on how to complete and submit the form, types of transcripts available, and who to contact with questions.

Students with Disabilities

The UCLA Center for Prehospital Care may provide reasonable accommodations during a program to students with disabilities (including learning disabilities). Reasonable accommodations are defined as accommodations that do not fundamentally alter the program. All students are required to meet the program's enrollment requirements.

If you require special accommodations, please submit your written request along with your program registration. Upon receipt, you will be contacted by our Client Services Manager to discuss your request and obtain your current disability evaluation (the evaluation must include a comprehensive diagnosis and testing conducted during adulthood or within 3 years of the student's initial request for disability-related services].
